The course follows and endorses current NHS Patient Safety Incident Investigation (PSII) guidance and teaches recognised good practice
approaches to engage and communicate with all those involved in healthcare investigations whilst considering Human Factors and
supporting families and staff as well as service users.
This course is designed to provide delegates with the tools and knowledge required to understand “Just Culture” (No Blame) and the “Duty
of Candour”, how to communicate effectively with patients and their families, and how to conduct the new “Being Open Conversations” as
well as more formal interviews and how to offer supportive feedback to all parties involved.
Whist the collection of statements is not required for any learning response method under PSIRF, a system-based method using interviews,
observations, cognitive walk-throughs etc, is used for information gathering. However, as witness statements will still likely be required for
an Inquest (outside of PSFIR) the writing of a legally robust statement is included in the course.
This one-day intensive masterclass is based around case scenarios, and we have been fortunate to partner with the Charity “Making
Families Count” to deliver this programme in a way that ensures the voice of families are heard. Our trainers have a background of working
in healthcare at a senior level, legal training and in working for NHS England.

• To understand the requirements of the new Patient Incident Response Framework (PSIRF) and the difference with the previous
system in particular the duty to involve patients. Families/carers and Staff.

• How to communicate effectively and with openness & sensitivity including how to “say sorry”
• Managing expectations withing the process
• How to conduct “no Blame interviews” under the PSIRF
• How to write a witness statement if required for an Inquest
• How to conduct a debrief and support staff
• Signposting for patients, families and staff and the use of advocacy.
